I've confirmed that this issue seems to happen around trees, and I found a good way to repeat it for testing. First, set up a series of pockets so that you can exit into a jungle biome, which tends to have lots of foliage on the ground. Stop the game while inside a pocket dimension and delete the Overworld so that the jungle will have to regenerate when you next enter its chunks. Reload the game and use an exit door. With some luck, you should end up stuck in the ground. I've also come up with a potential solution but it needs some testing.
